Union Authorization Cards

Did you sign a union authorization card or submit a form online without fully realizing what it was? Or did you sign one and change your mind? Whatever the reason, if you signed a card and want it back, here are some steps you can take to try and revoke your card from the union.

Email the union and ask for your card back. Include your name, our hospital name and your email or phone number.

To Whom It May Concern:

I am revoking my authorization for United Food & Commercial Workers Local 75 (UFCW Local 75) to represent me for the purposes of collective bargaining and resigning my union membership. Please return my authorization card to the email address below. Additionally, please remove my name from future UFCW Local 75 outreach efforts and do not sell, rent or otherwise share my personal information with any other labor union or organization.

If the union does not agree, you can send a certified letter using the same template above to its headquarters and a copy to the National Labor Relations Board Region 09.
